While these symptomscontinue, the pains and contractions foreboding the approachingexpulsion of the foetus set in, and the hemorrhage takes place. After the blood has begun to flow the course of the trouble ispretty much the same in every case. If the foetus is separated frthe organism of the mother, it has to be expelled. If the hemorrhage is at all copious, the expulsion can no longer be preventedThe course of the diflaculty now varies according to the differenstates of the uterus. During the first months of pregnancy thelower segment is still very rigid, the os tincse firmly closed ;force or a longer time is required to open the womb, whereas in tsubsequent months the vaginal portion becomes more yielding anddilatable. Hence a miscarriage extends over a longer space of timin the commencement than in the subsequent months of pregnancy ;moreover, the hemorrhage in the former case is more profuse. Asa rule the ovum, up to the third month, is discharged uninjured, sometimes unperceived, whereas, at a subsequent period, the membranes of the ovum are ruptured, and afterwards expelled subsequently to the expulsion of the foetus. The constitutional symptoms are preeminently those of acute aneemia ; even the convulsive phenomena which are not unfrequently present, are onlj^ occasioned by the ansemic condition of the patient, very seldom by thamount of pain involved in the hemorrhage. Primiparoegenerallysuffer most. As soon as the foetus is expelled, the flow of bloodgenerally ceases very soon. But if the placenta has already beenformed, the whole of the foetus is not always expelled, but fragmentB of the ovum remain in the uterus and, by preventing the fulcontraction of this organ, cause the hemorrhage to continue. Thisis so much more dangerous as it is commonly more copious than atthe commencement of miscarriage, the danger being still greater,if we are not able to remove the remnants of the foetus. In thiscase the hemorrhage may likewise be internal, if the foetal remnaclose up the os tincse. Hemorrhages of this kind are always moreor less threatening, and sometimes endanger life. What we have72 Diseases of the Female Sexual Organs.said, shows that miscarriage is least apt to occur in the first tmonths; and that the danger involved in the hemorrhage is greatest, if a placenta has already formed, because remnants of the ovare eiisily retained in the uterus. The whole course of such an accident assumes a peculiar form^if the uterus firmly contracts round the remnants of the ovum. Insuch a case the flow of blood suddenly stops and complete recoverhttp://www.archive.org/stream/sciencetherapeu00kafkgoog/sciencetherapeu00kafkgoog_djvu.txt
