Chirurgia mini-invasiva del rachide lombare<br />
(apofisi spinose e legamenti connessi) e, soprattutto se effettuate<br />
con il microscopio operatorio, perché evitano resezioni eccessive<br />
delle articolazioni posteriori. L’evoluzione della laminotomia è la<br />
decompressione bilaterale con accesso monolaterale, che preserva<br />
ulteriormente la stabilità poiché comporta un’asportazione molto<br />
parziale delle apofisi articolari nel lato opposto a quello della<br />
laminotomia.<br />
Gli spaziatori interspinosi sono sicuramente presi<strong>di</strong> utili nel trattamento<br />
<strong>di</strong> varie patologie, quali <strong>di</strong>scopatie degenerative con altezza<br />
<strong>di</strong>scale non eccessivamente ridotta, stenosi moderate o patologie<br />
faccettali moderate. Ciò anche per la semplicità e rapi<strong>di</strong>tà <strong>di</strong> applicazione.<br />
Tuttavia, l’uso eccessivo e spesso in<strong>di</strong>scriminato che <strong>di</strong><br />
essi ancora si fa non ha consentito <strong>di</strong> in<strong>di</strong>viduare le in<strong>di</strong>cazioni<br />
elettive al loro uso e ha portato forse ad “inquinare” i risultati.<br />
Seppure potenzialmente utili, questi presi<strong>di</strong> devono essere ancora<br />
stu<strong>di</strong>ati rigorosamente per in<strong>di</strong>viduarne i limiti.<br />
Le stabilizzazioni a viti peduncolari percutanee hanno rappresentato<br />
un significativo passo avanti nella riduzione dell’invasività<br />
delle stabilizzazioni. Esse sono valide soprattutto quando non si<br />
devono associare altri atti chirurgici (fratture vertebrali), ma lo<br />
sono anche quando si deve effettuare una decompressione delle<br />
strutture nervose o un’artrodesi intersomatica, se ciò viene fatto<br />
con un accesso monolaterale (decompressione bilaterale con<br />
accesso monolaterale, TLIF)<br />
Negli ultimi anni vi è stato un eccesso, talora smisurato, nel trattamento<br />
delle fratture osteoporotiche con vertebro-cifoplastica,<br />
<strong>di</strong>menticando che la frattura guarisce comunque nel corso <strong>di</strong> alcuni<br />
mesi e che il dolore può essere ridotto con un corsetto. Nonostante<br />
ciò, queste meto<strong>di</strong>che possono essere <strong>di</strong> grande utilità nel paziente<br />
osteoporotico anziano che abbia un marcato dolore vertebrale, e in<br />
cui la vertebra fratturata tende spesso a cuneizzarsi nel corso <strong>di</strong> 3-5<br />
settimane, causando un’ ipercifosi (dorsale) o cifosi (lombare) che<br />
possono essere causa <strong>di</strong> dolore persistente nel tempo.<br />
