Noi abbiamo ripreso l’esperienza riportata da Vandenbussche 15 il<br />
quale pubblicò pochissimi casi <strong>di</strong> fratture a tre e quattro parti impattando<br />
tra i frammenti dell’omero osso autologo tricorticale prelevato<br />
dalla cresta iliaca e effettuando una osteosintesi con suture.<br />
Cinque pazienti hanno avuto un buon risultato ma nella pubblicazione<br />
non viene chiarita la modalità <strong>di</strong> posizionamento dell’innesto<br />
né lo spostamento dei frammenti e la loro <strong>di</strong>slocazione.<br />
Abbiamo mo<strong>di</strong>ficato questa tecnica per due concetti che noi consideriamo<br />
innovativi: il primo è che nel riposizionare i frammenti<br />
nella corretta anatomia ricostruendo anche la normale retro torsione<br />
omerale e della calotta, il minus sotto tuberositario va ricostituito<br />
con osso modellato a forma trapezoidale triangolare in modo che i<br />
tre vertici del poliedro siano <strong>di</strong> supporto alle pareti e alla calotta; il<br />
secondo è il concetto che noi abbiamo definito come “ ricostruzione<br />
me<strong>di</strong>ale del calcar” e cioè la necessità <strong>di</strong> riparazione perfetta del<br />
calcar a supporto della calotta cefalica nella corretta torsione che<br />
rappresenta l’unica possibilità <strong>di</strong> ricostituire l’anatomia e <strong>di</strong> consentire<br />
la rivascolarizzazione 6 .<br />
Noi cre<strong>di</strong>amo che il trattamento più corretto nelle fratture a 4 frammenti<br />
e nelle fratture complesse con per<strong>di</strong>ta <strong>di</strong> sostanza, sia l’osteosintesi<br />
con ricostruzione me<strong>di</strong>ale ed innesto <strong>di</strong> osso tricorticale<br />
o l’ausilio <strong>di</strong> spaziatori come il sistema Da Vinci, che consente <strong>di</strong><br />
mantenere una buona riduzione della frattura colmando il vuoto<br />
metaipifisario e favorendo la rivascolarizzazione della testa infatti<br />
abbiamo raccolto una serie <strong>di</strong> pazienti che anche se limitata avendo<br />
raggiunto un follow-up <strong>di</strong> 10 anni ha <strong>di</strong>mostrato che la testa omerale<br />
può essere salvata, ha possibilità <strong>di</strong> neovascolarizzazione e <strong>di</strong><br />
guarigione con ottimi risultati anche a <strong>di</strong>stanza 7 . La costante valutazione<br />
<strong>di</strong> questi casi il miglioramento dei materiali, l’innovazione<br />
dell’uso dell’osso <strong>di</strong> banca e <strong>di</strong> altri biomateriali ci ha consentito<br />
anche <strong>di</strong> migliorare i risultati rispetto a molte casistiche pubblicate<br />
sia in termini <strong>di</strong> risultato funzionale che in termini <strong>di</strong> reinterventi.<br />
Infatti i nostri re interventi per complicanze tecniche rispetto alla<br />
letteratura la cui me<strong>di</strong>a raggiunge anche il 40% sono stati 2 su 507<br />
che rappresentano lo 0,4%.<br />
