What’s new in acute osteo articular hematogenus infections of the child?<br />

SuMMary<br />

Acute osteoarticular hematogenous infections of the child are a<br />

serious <strong>di</strong>sease. Their incidence remains stable even in countries<br />

with a high socioeconomic level.<br />

A new approach of this <strong>di</strong>sease is necessary due to appearance<br />

of new clinical forms. In fact, new causal bacteriological strains<br />

have appeared. This is also probably due to many factors, of<br />

which:<br />

• The change of our environment and of our food habits polluted<br />

by veterinary bree<strong>di</strong>ng using antibiotics.<br />

• The inadequate use of antibiotics without bacteriological proof.<br />

The saying «The slightest doubt over the <strong>di</strong>agnosis of acute<br />

osteomyelitis has to lead to starting the antibiotic treatment in the<br />

following quarter of hour and immobilization in the next hour»<br />

is no longer necessary. We actually have more means to prove a<br />

bacteriological infection:<br />

• The bacteriologic techniques have developed thus allowing to<br />

incriminate more frequently other germs like Kingella Kingea.<br />

The staphylococcus aureus remains the most frequently found<br />

pathogen with the emergence of new community-acquired<br />

methicillin resistant strains, particularly more virulent. With<br />

the development of molecular biology, we have been able to<br />

bring to light the fact that this virulence is due to the secretion<br />

by these strains of a new toxin, The Panton Valentine<br />

Leucoci<strong>di</strong>n. It’s responsible of serious clinical pictures with<br />

frequent complications such as deep venous thrombosis.<br />

• MRI is more and more performing thus giving more sensitive<br />

«biological images»<br />

The absence of signal mo<strong>di</strong>fication allows to eliminate bacteriologic<br />

infection.<br />

The new profile of this sickness alters our therapeutic attitude with<br />

a tendency to shorten the antibiotic treatment while maintaining<br />

the same efficiency.<br />
