L’osteosintesi stabile delle fratture del ra<strong>di</strong>o <strong>di</strong>stale ha avuto in<br />
anni recenti una <strong>di</strong>ffusione più ampia grazie all’introduzione <strong>di</strong><br />
mezzi <strong>di</strong> sintesi <strong>di</strong> nuova generazione. Le placche attualmente in<br />
uso adottano i concetti della stabilità angolare, del sostegno subcondrale<br />
dei frammenti epifisari e della multi<strong>di</strong>rezionalità delle<br />
viti. Il basso profilo e la varietà dei <strong>di</strong>segni permettono <strong>di</strong> adattare<br />
i mezzi <strong>di</strong> sintesi al tipo <strong>di</strong> frattura e consentono sia un approccio<br />
volare che dorsale, in casi selezionati. L’approccio volare permette<br />
<strong>di</strong> trattare la maggior parte delle fratture. Alcuni particolari tecnica<br />
sono fondamentali per ottenere una buona riduzione e prevenire<br />
complicanze. In casi selezionati può essere utile un approccio<br />
dorsale che, con i nuovi mezzi <strong>di</strong> sintesi, ha un ridotto impatto sui<br />
ten<strong>di</strong>ni estensori.La nostra esperienza è basata su 153 fratture del<br />
ra<strong>di</strong>o <strong>di</strong>stale trattate con placca in 4 anni (2006-2009). L’85% delle<br />
fratture erano articolari. Negli 86 pazienti sottoposti a revisione,<br />
con un follow-up me<strong>di</strong>o <strong>di</strong> 1 anno e due mesi, il valore me<strong>di</strong>o del<br />
DASH è stato <strong>di</strong> 4 (minimo 0, massimo 18). In base al Mayo Wrist<br />
Score il risultato è stato classificato come eccellente in 21 pazienti<br />
(24%), buono in 58 (67%), <strong>di</strong>screto in 7 (9%). L’osteosintesi con<br />
placche a stabilità angolare è un metodo efficace e affidabile <strong>di</strong><br />
trattamento delle fratture del ra<strong>di</strong>o <strong>di</strong>stale. In particolare, può<br />
essere oggi considerato il trattamento <strong>di</strong> elezione per le fratture<br />
articolari.<br />

SuMMary<br />
Internal fixation of fractures of the <strong>di</strong>stal ra<strong>di</strong>us has gained wider<br />
acceptance following the introduction of new fixation devices.<br />
Current plate fixation is based on the principles of angular stability,<br />
multi<strong>di</strong>rectional locking screws and subcondral support. Low<br />
profile plates and <strong>di</strong>fferent plate design allow both volar and dorsal<br />
approach. An accurate surgical technique allows to achieve a good<br />
reduction of most fractures by a volar approach. In a few selected<br />
fractures a dorsal plate can be used since new designs have reduced<br />
the risk of extensor tendon <strong>di</strong>sfunction or rupture. Our experience<br />
is based on 153 fractures of the <strong>di</strong>stal ra<strong>di</strong>us treated by plate<br />

(9%). Internal fixation by angular stability plates is an effective and<br />
reliable method of treatment of most wrist fractures and it should<br />
be considered the current treatment of choice for articular fractures<br />
of the <strong>di</strong>stal ra<strong>di</strong>us.<br />

INTrODuzIONE<br />
L’evoluzione del trattamento delle fratture del ra<strong>di</strong>o <strong>di</strong>stale negli<br />
ultimi 15 anni è stata caratterizzata da una maggiore accuratezza<br />
nella riduzione e dalla ricerca <strong>di</strong> mezzi <strong>di</strong> sintesi che consentissero<br />
una fissazione stabile senza troppo interferire con i ten<strong>di</strong>ni estensori<br />
e flessori.<br />
L’introduzione <strong>di</strong> placche a stabilità angolare a partire dal 2001<br />
ha consentito un progresso sostanziale nell’osteosintesi rigida<br />
delle fratture del ra<strong>di</strong>o <strong>di</strong>stale, tanto da ampliarne le in<strong>di</strong>cazioni e<br />
mo<strong>di</strong>ficare i protocolli <strong>di</strong> trattamento, con una sempre maggiore<br />
<strong>di</strong>ffusione dell’osteosintesi con placca rispetto ad altri trattamenti<br />
chirurgici 1 . Le fratture articolari, che richiedono la maggiore accuratezza<br />
<strong>di</strong> riduzione, sono quelle che più spesso vengono trattate<br />
con una osteosintesi con placca.<br />
I MEzzI DI SINTESI<br />
Le placche attualmente <strong>di</strong>sponibili realizzano alcuni principi che<br />
hanno profondamente mo<strong>di</strong>ficato l’osteosintesi del ra<strong>di</strong>o <strong>di</strong>stale.<br />
La stabilità angolare<br />
La fissazione delle viti alla placca, secondo una meto<strong>di</strong>ca e una<br />
tecnologia già applicate in altri <strong>di</strong>stretti, ha trovato nel polso<br />
un’applicazione utilissima, dal momento che la comminuzione<br />
della corticale dorsale impe<strong>di</strong>sce in molti casi una vera fissazione<br />
interframmentaria. Le viti “bloccate” forniscono sostegno all’epifisi<br />
e creano con la placca un “ponte” che scarica la zona metafisaria<br />
dorsale, deviando le forze sulla placca volare che funziona da<br />
bypass meccanico.<br />
Il sostegno subcondrale<br />
Questo concetto è strettamente collegato a quello della stabilità<br />
angolare. Infatti non potendosi realizzare in molti casi una fissazione<br />
interframmentaria, a causa della comminuzione dorsale,<br />
