Umstellungsstrategien randomisiert: Titration <strong>in</strong> aufsteigen<strong>der</strong><br />
Dosierung (5 mg / Tag bis 15 mg / Tag) o<strong>der</strong> Umstellung auf e<strong>in</strong>e<br />
fixe Dosis (15 mg / Tag). Ab <strong>der</strong> 6. Woche konnte jeweils e<strong>in</strong>e Dosisanpassung<br />
im Rahmen von 10 – 30 mg / Tag vorgenommen werden.<br />
Die mittleren Verän<strong>der</strong>ungen <strong>der</strong> GEOPTE- sowie <strong>der</strong> PANSS-<br />
Subskala-Scores wurden unter Anwendung deskriptiver statistischer<br />
Methoden (95 % Konfidenz<strong>in</strong>tervalle) ausgewertet.<br />
Diskussion / Ergebnisse: Die GEOPTE-Skala besteht aus 15 Items.<br />
Sie erfasst Defizite, die von den Patienten o<strong>der</strong> den betreuenden<br />
Personen wahrgenommen werden. E<strong>in</strong>e negative Verän<strong>der</strong>ung <strong>der</strong><br />
Scores bedeutet e<strong>in</strong>e Verbesserung. GEOPTE-Summenscores und<br />
PANSS-Kognitionssubskalen-Scores nahmen <strong>in</strong> <strong>der</strong> 4. und 12. Woche<br />
unabhängig von <strong>der</strong> Umstellungsstrategie ab (LOC). Die Verän<strong>der</strong>ungen<br />
<strong>der</strong> GEOPTE-Summenscores <strong>in</strong> <strong>der</strong> 12. Woche gegenüber<br />
<strong>der</strong> Basel<strong>in</strong>e betrugen gemäß <strong>der</strong> Patientene<strong>in</strong>schätzung bei<br />
Dosistitrations- bzw. Fixdosis-Umstellungsstrategie -5,27 (n=194)<br />
bzw. -6,12 (n=191) und -4,17 (n=98) bzw. -7,19 (n=95) gemäß Bewertung<br />
durch die Betreuenden. Aripiprazol wurde gut vertragen,<br />
am häufigsten wurde über Schlaflosigkeit als Nebenwirkung berichtet<br />
(8,5 %, n=34/399).<br />

„Theory of M<strong>in</strong>d“ bei Schizophrenie: E<strong>in</strong>e fMRI-Studie zur Attribution<br />
von Überzeugungen<br />

E<strong>in</strong>leitung: „Theory of M<strong>in</strong>d“ (ToM) bezieht sich auf die Fähigkeit,<br />
sich selbst und an<strong>der</strong>en mentale Zustände wie Wahrnehmungen,<br />
Wünsche, Intentionen, Emotionen und Überzeugungen zuzuschreiben.<br />
Diese Fähigkeit ist bei Schizophreniepatienten bee<strong>in</strong>trächtigt.<br />
Neben Verhaltensdefiziten zeigen Schizophrene verän<strong>der</strong>te<br />
neuronale Aktivierungen h<strong>in</strong>sichtlich <strong>der</strong> an <strong>der</strong> ToM<br />
beteiligten Gehirnarealen, v. a. <strong>in</strong> präfrontalen Regionen. Bislang<br />
ist ungeklärt, welche ToM-Komponenten betroffen s<strong>in</strong>d. Die vorliegende<br />
Studie fokussiert e<strong>in</strong>e spezifische ToM-Komponente, die<br />
Zuschreibung von Überzeugungen.<br />
Methode: Die Fähigkeit, mentale Zustände an<strong>der</strong>er Menschen zu<br />
verstehen, <strong>der</strong>en Überzeugungen nicht mit <strong>der</strong> Realität übere<strong>in</strong>stimmen,<br />
wurde anhand von ToM-Bil<strong>der</strong>geschichten getestet. Mittels<br />
funktioneller Magnetresonaztomographie (fMRT) wurden die<br />
neuronalen Korrelate <strong>der</strong> Überzeugungsattribution bei 13 Schizophreniepatienten<br />
im Vergleich zu 13 gesunden Kontrollprobanden<br />
untersucht.<br />
Diskussion / Ergebnisse: H<strong>in</strong>sichtlich <strong>der</strong> Verhaltensdaten ergaben<br />
sich ke<strong>in</strong>e Unterschiede zwischen Patienten und Kontrollen. Auf<br />
neuronaler Ebene zeigten die Schizophrenen e<strong>in</strong> verän<strong>der</strong>tes Aktivierungsmuster.<br />
Erwartungsgemäß aktivierten die gesunden<br />
Probanden die zu den klassischen ToM-Arealen zählenden medialpräfrontalen<br />
Regionen (BA 10, 6, 8) sowie e<strong>in</strong>en Bereich im<br />
temporoparietalen Kortex (BA 40). Bei den Schizophrenen fehlten<br />
entsprechende Aktivierungen. Sie aktivierten dagegen <strong>in</strong>feriorfrontale<br />
Regionen (BA 44, 45, 47), laterale Anteile des superiorfrontalen<br />
Kortex (BA 6) sowie laterale Anteile des dorsalen Präfrontalkortex<br />
(BA 9). Die Ergebnisse dieser Studie deuten darauf<br />
h<strong>in</strong>, dass schizophrene Patienten <strong>in</strong> <strong>der</strong> Lage s<strong>in</strong>d, durch e<strong>in</strong>en<br />
Kompensationsmechanismus, <strong>der</strong> durch e<strong>in</strong> verän<strong>der</strong>tes neuronales<br />
Aktivierungsmuster zum Ausdruck kommt, e<strong>in</strong>fache ToM-Aufgaben<br />
zu lösen. Dabei sche<strong>in</strong>en Arbeitsgedächtnis sowie <strong>in</strong>hibitorische<br />
Kontrollfunktionen stärker beansprucht zu werden als bei<br />
Gesunden. Vorhergehende Untersuchungen haben gezeigt, dass <strong>der</strong>artige<br />
Kompensationsstrategien bei komplexeren ToM-Aufgaben<br />
nicht mehr erfolgreich s<strong>in</strong>d. Wie durch therapeutische Interventionen<br />
die ToM-Fähigkeit und damit verbundene soziale Kompetenzen<br />
verbessert werden können, sollte Gegenstand weiterer Forschung<br />
se<strong>in</strong>.<br />

Self monitor<strong>in</strong>g deficit might be related to shorten<strong>in</strong>g of microstate<br />
D – A rest<strong>in</strong>g state EEG study <strong>in</strong> auditory verbal halluc<strong>in</strong>ations<br />

Introduction: All subjective experiences and eventually result<strong>in</strong>g<br />
overt behaviour result from an <strong>in</strong>teraction of the subject‘s <strong>in</strong>ternal<br />
bra<strong>in</strong> state with environmental stimuli. This is true <strong>in</strong> normal bra<strong>in</strong><br />
processes, but also applies to pathological processes. Us<strong>in</strong>g EEG,<br />
this study <strong>in</strong>vestigated whether abnormal perceptions and cognitions<br />
<strong>in</strong> schizophrenia might be related to abnormal rest<strong>in</strong>g states<br />
of the bra<strong>in</strong>. Previous research has shown that a specific class (class<br />
D) of sub-second bra<strong>in</strong> states (so-called EEG microstates) is consistently<br />
shorter <strong>in</strong> productive schizophrenic patients and that the<br />
shorten<strong>in</strong>g was correlated to positive psychotic symptoms. The aim<br />
of the present study was to relate microstate class D duration to<br />
spontaneous with<strong>in</strong>-subject fluctuations of auditory halluc<strong>in</strong>atory<br />
experiences. The hypothesis was that state D is shorter dur<strong>in</strong>g halluc<strong>in</strong>ations<br />
than <strong>in</strong> the absence of halluc<strong>in</strong>ations.<br />
Method: EEGs of n<strong>in</strong>e right handed schizophrenic subjects with<br />
acute auditory verbal halluc<strong>in</strong>ations (AVH) were recorded. Subjects<br />
were <strong>in</strong>structed to listen and attend to their voices and to <strong>in</strong>dicate<br />
the beg<strong>in</strong>n<strong>in</strong>g and end<strong>in</strong>g of them each by a button press. Off l<strong>in</strong>e,<br />
EEG was divided <strong>in</strong>to periods with and without AVH. Microstates<br />
were analysed separately for each period. Mean microstate duration,<br />
mean number of microstates per second and percentage of total<br />
analysis time occupied <strong>in</strong> that state were computed follow<strong>in</strong>g our<br />
standard procedure.<br />
Discussion / Results: Four microstate classes (A, B, C, and D) accounted<br />
for 79.1 % of the data variance. Duration of microstate D<br />
was significantly shorter (p=0.046) <strong>in</strong> periods with AVH compared<br />
to periods without AVH. For AVH, the common hypotheses suggested<br />
deficient self monitor<strong>in</strong>g lead<strong>in</strong>g to a misattribution of <strong>in</strong>ternal<br />
and external stimuli. We hypothesize that microstate D has<br />
relevant self-monitor<strong>in</strong>g functions and its premature term<strong>in</strong>ation<br />
may contribute to the erroneous conclusion <strong>in</strong> AVH that self-generated<br />
<strong>in</strong>ner speech comes from an external source. The reduced<br />
stability of rest<strong>in</strong>g state networks fits well with the disconnection<br />
syndrome hypothesis of schizophrenia.<br />
