Introduction: Patients with depression, anxiety disor<strong>der</strong>s and<br />
OCD have been studied before and after psychotherapy by fMRT<br />
and PET. Control conditions <strong>in</strong>cluded normal subjects and pharmacological<br />
treatments.<br />
Method: Studies of bra<strong>in</strong> imag<strong>in</strong>g <strong>in</strong> psychotherapy are reviewed<br />
and discussed regard<strong>in</strong>g methodological and cl<strong>in</strong>ical implications.<br />
Discussion / Results: Studies have identified changes <strong>in</strong> bra<strong>in</strong> activation<br />
patterns <strong>in</strong> areas associated with emogtion regulation, particularly<br />
<strong>in</strong> treatment respon<strong>der</strong>s, which were comparable to effective<br />
pharmacological treatments. Limitations perta<strong>in</strong> to number of<br />
studies, sample sizes and brevity of treatments. Conclusions Based<br />
on the studies presented and the emerg<strong>in</strong>g view of the plasticity of<br />
the bra<strong>in</strong> the dist<strong>in</strong>ction between somatic treatments hav<strong>in</strong>g an impact<br />
on the bra<strong>in</strong> and psychological treatments with more elusive<br />
subjective effects has become obsolete. To date, implications for<br />
psychotherapeutic practise are limited. Yet, functional neuroimag<strong>in</strong>g<br />
is a promis<strong>in</strong>g approach to study issues relevant for psychotherapy,<br />
e.g. biological bases of mental disor<strong>der</strong>s, prognostic factors<br />
and mechanisms of psychotherapeutic change.<br />

To date, cognitive behavioural therapy (CBT) is be<strong>in</strong>g recognized as<br />
an important treatment <strong>in</strong> conjunction with psychopharmacotherapy<br />
<strong>in</strong> schizophrenia. However, there is only very scarce research<br />
on the effects of such <strong>in</strong>terventions on bra<strong>in</strong> function. Recent studies<br />
have suggested that jump<strong>in</strong>g to conclusions is a predom<strong>in</strong>ant<br />
cognitive style <strong>in</strong> patients which might lead to the development of<br />
delusions. In this BMBF funded multi-centre trial, we <strong>in</strong>vestigated<br />
the effect of CBT on neural correlates of “jump<strong>in</strong>g to conclusions”<br />
and the “attributional style” that patients with schizophrenia exhibit.<br />
Patients and control subjects were recruited <strong>in</strong> six centres and<br />
measured with 3-Tesla functional magnetic imag<strong>in</strong>g (fMRI) before<br />
and after 9 months of cognitive behavioural therapy. It could be<br />
shown that CBT ameliorates differences <strong>in</strong> bra<strong>in</strong> activations between<br />
patients and controls after n<strong>in</strong>e months. These results support<br />
the feasibility of fMRI multicenter trials and sheds further<br />
light <strong>in</strong>to the mechanisms relat<strong>in</strong>g psychotherapy to bra<strong>in</strong> function.<br />

E<strong>in</strong>leitung: E<strong>in</strong>e Panikstörung führt bei den Betroffenen zu erheblichen<br />
Leiden und hat gesellschaftlich große sozioökonomische<br />
Folgen. Die neurobiologische Forschung konnte e<strong>in</strong> Netzwerk zerebraler<br />
Regionen identifizieren, das <strong>in</strong> <strong>der</strong> Pathophysiogie <strong>der</strong><br />
Panikstörung e<strong>in</strong>e große Rolle spielt und dem u. a. Amygdala, Hippocampus,<br />
C<strong>in</strong>gulum und Insula angehören. E<strong>in</strong>e abnorme Informationsverarbeitung<br />
<strong>in</strong> diesem Netzwerk kann zu e<strong>in</strong>er panikauslösenden<br />
Fehl<strong>in</strong>terpretation von Stimuli führen, die e<strong>in</strong>e <strong>der</strong><br />
kl<strong>in</strong>ischen Hauptsymptome <strong>der</strong> Panikstörung darstellt. Medikamentöse<br />
Behandlung und Psychotherapien können dieses Angstnetzwerk<br />
modulieren.<br />
Methode: Dies wird u. a. im Rahmen e<strong>in</strong>es vom BMBF f<strong>in</strong>anzierten<br />
Projektes untersucht, <strong>in</strong> dem e<strong>in</strong>e große Anzahl von Patienten multizentrisch<br />
vor und nach e<strong>in</strong>er manualisierten Psychotherapie und<br />
mittels funktioneller Magnetresonanztomographie untersucht werden.<br />
Diskussion / Ergebnisse: Aktuelle Ergebnisse zur Untersuchung<br />
<strong>der</strong> neuronalen Korrelate <strong>der</strong> Panikstörung werden vorgestellt und<br />
die Auswirkungen auf aktuelle neurobiologische Störungsmodelle<br />
und Therapiemöglichkeiten <strong>der</strong> Panikstörung diskutiert.<br />

E<strong>in</strong>leitung: Meta-Analysen zu Kognitiver Verhaltenstherapie (KVT)<br />
zeigen, dass KVT die Negativ-Symptomatik bei schizophrenen Psychosen<br />
reduzieren kann. Allerd<strong>in</strong>gs s<strong>in</strong>d die meisten Studien, die<br />
hier zugrundegelegt werden, auf die Positivsymptomatik ausgerichtet.<br />
Randomisierte kl<strong>in</strong>ische Studien zur Negativsymptomatik<br />
fehlen bislang.<br />
Methode: Die Studie ist multicentrisch, e<strong>in</strong>fach bl<strong>in</strong>d und randomisiert<br />
(ISRCTN25455020). Die Wirksamkeit <strong>der</strong> KVT wird mit<br />
Kognitiver Remediation (KR) verglichen. KR wurde als Kompara-<br />
