Punches of various bra<strong>in</strong> regions were taken for BDNF evaluation<br />
by ELISA.<br />
Discussion / Results: ECT and SCES of the ventral PLC (vPLC)<br />
and NAC, but not dorsal PLC (dPLC), significantly <strong>in</strong>creased sucrose<br />
preference <strong>in</strong> CMS animals. In the sexual behavior test the<br />
number of mounts <strong>in</strong> CMS animals was significantly lower than<br />
controls. SCES treatment of the NAC <strong>in</strong>creased the sexual drive. In<br />
a sexual partner preference test control animals preferred to mount<br />
more often the new stimulus female compared to their previous sexual<br />
partner while CMS did not show such preference. In the exploration<br />
test, CMS <strong>in</strong>duced a reduction <strong>in</strong> the number of center visits,<br />
while ECT (but not SCES) normalized this reduction. F<strong>in</strong>ally, ECT<br />
(but not SCES) <strong>in</strong>duced impairment <strong>in</strong> the learn<strong>in</strong>g and memory as<br />
<strong>in</strong>dicated by the Morris water maze. ECT and SCES <strong>in</strong>creased<br />
BDNF level <strong>in</strong> dPLC and <strong>in</strong> the striatum.This study implicates the<br />
ventral PLC and the NAC <strong>in</strong> the pathophysiology of depressive behavior<br />
and suggests that SCES of these regions can <strong>in</strong>duce an antidepressant<br />
effect similar to ECT, but without any cognitive impairment.<br />
In addition these results suggest that SCES of the NAC is an<br />
effective approach for treat<strong>in</strong>g reduced sexual drive <strong>in</strong>duced by<br />
chronic stress.<br />

E<strong>in</strong>leitung: Bis ca. Mitte <strong>der</strong> 60er Jahre herrschte <strong>in</strong> <strong>der</strong> Depressionsforschung<br />
das psychoanalytische Denken vor. In den 70er Jahren<br />
wurden vor allem l<strong>in</strong>eare Theorien aufgestellt, die experimentell<br />
untersucht wurden. Diese Modelle untersuchten meist korrelativ<br />
Wenn-Dann-Hypothesen. Erst <strong>in</strong> den 80er Jahren wurden e<strong>in</strong>dimensionale<br />
Hypothesen durch multifaktorielle, <strong>in</strong>tegrative Modelle<br />
ersetzt. Gängige Depressionsmodelle zeigen den Weg <strong>in</strong> die Depression<br />
– aber nicht den Weg aus <strong>der</strong> Depression. Häufig greifen<br />
diese Modelle nur e<strong>in</strong>en möglichen Aspekt heraus o<strong>der</strong> sie s<strong>in</strong>d zu<br />
allgeme<strong>in</strong> formuliert (Bsp.: Diathese-Stress-Modell). Die Synergetik<br />
wird nicht berücksichtigt und oft werden nur e<strong>in</strong>dimensionale<br />
Wenn-Dann-Erklärungen verwendet. Ausserdem wird Depression<br />
häufig als Variable des Systems und nicht als Zustand des Systems<br />
selbst behandelt.<br />
Methode: Im ersten Teil wird e<strong>in</strong> neues Depressionsmodell vorgestellt.<br />
Anhand dieses Modelles wird gesundes und depressives Verhalten,<br />
sowie <strong>der</strong> Weg <strong>in</strong> die Depression und <strong>in</strong>sbeson<strong>der</strong>e <strong>der</strong> Weg<br />
aus <strong>der</strong> Depression im Computer simuliert. Auswirkungen verschiedener<br />
Therapiemöglichkeiten werden diskutiert. Auf die mathematische<br />
Formulierung des Modells, <strong>in</strong>sbeson<strong>der</strong>e auf die Differentialgleichung<br />
<strong>der</strong> Depression wird kurz e<strong>in</strong>gegangen.<br />
Diskussion / Ergebnisse: Im zweiten Teil wird die Umsetzung dieses<br />
Modelles <strong>in</strong> Komb<strong>in</strong>ation mit IPT, CBASP und Komplementärmediz<strong>in</strong><br />
auf <strong>der</strong> Spezialstation für Depressionsbehandlung und<br />
Psychotherapie <strong>der</strong> Psychiatrischen Kl<strong>in</strong>ik Zugersee, Schweiz, vorgestellt<br />
und diskutiert. IPT als State-of-the-Art psychotherapeutischer<br />
Behandlung episodischer Depressionen und CBASP als<br />
State-of-the-Art psychotherapeutischer Behandlung chronischer<br />
Depressionen werden kurz vorgestellt. Komplementärmediz<strong>in</strong>ische<br />
Massnahmen bieten darüber h<strong>in</strong>aus Möglichkeiten <strong>der</strong> <strong>in</strong>terdiszipl<strong>in</strong>ären<br />
positiven E<strong>in</strong>flussnahme auf das begleitende somatische<br />
Syndrom und Verbesserung <strong>der</strong> Lebensqualität (QoL).<br />

Treat<strong>in</strong>g depression and suicidal crises more effectively via<br />
screen<strong>in</strong>g patients for overexcitabilities and the „third factor“<br />

Introduction: This paper argues that gifted persons who suffer<br />
from depression or suicidal symptoms, should not be offered traditional<br />
therapeutic methods only. These methods may not be effective<br />
for these persons <strong>in</strong> the long run. Instead, these <strong>in</strong>dividuals<br />
should be provided with a treatment based on the Theory of Positive<br />
Dis<strong>in</strong>tegration (TPD), a theory of personality development<br />
devised by the Polish psychiatrist and psychologist Kazimierz<br />
Dabrowski (1902 – 1980).<br />
Method: The paper describes the TPD and draws implications for<br />
the treatment of gifted patients. Based on empirical observations,<br />
the TPD views depression and suicidal conflicts, if experienced by<br />
„gifted“ persons, not per se as an illness. Instead, it regards these<br />
symptoms as a necessary phase <strong>in</strong> which given socially / biologically<br />
determ<strong>in</strong>ed mental structures dis<strong>in</strong>tegrate, so that a more conscious<br />
and autonomous personality structure can develop. „Gifted“ means<br />
the respective person possesses (1) an <strong>in</strong>ner autonomous drive to<br />
develop one‘s own personality (called ‚third factor‘), (2) special abilities<br />
and talents and (3) overexcitabilities (psychomotor, sensual,<br />
<strong>in</strong>tellectual, imag<strong>in</strong>ational, emotional). The overexcitabilities make<br />
these people especially prone to dis<strong>in</strong>tegration. At the same time,<br />
their characteristics can enable these <strong>in</strong>dividuals to overcome their<br />
neurotic symptoms and to achieve ‚higher‘ levels of personality<br />
development, characterized by altruism, wisdom and contentment.<br />
Discussion / Results: The ma<strong>in</strong> therapeutic <strong>in</strong>tervention follow<strong>in</strong>g<br />
from the TPD is to help the clients reframe their <strong>in</strong>tense feel<strong>in</strong>gs<br />
and <strong>in</strong>ner conflicts as creative and developmental <strong>in</strong> nature, so that<br />
they can move ahead, not fall apart. In addition, autotherapy should<br />
be applied, which provides the clients with a long-term tool to deal<br />
with possible future symptoms of dis<strong>in</strong>tegration. By means of a selfassessment<br />
questionnaire, therapists can identify persons who exhibit<br />
high overexcitabilities and the „third factor“, and who may<br />
therefore profit from the TPD-approach. Moreover, the questionnaire<br />
provides a basis for empirically test<strong>in</strong>g the therapeutic<br />
effectiveness of the TPD-approach.<br />

EEG basierte Vigilanzregulation bei Patienten mit Depressiver<br />
Störung vor und nach therapeutischem Schlafentzug<br />

E<strong>in</strong>leitung: In kl<strong>in</strong>ischen Studien konnte gezeigt werden, dass<br />
Schlafentzug (SE) e<strong>in</strong>en antidepressiven Effekt bei Patienten mit<br />
depressiver Störung haben kann. Die Freisetzung von adrenergen<br />
Transmittern durch physische Aktivität während des SE könnte e<strong>in</strong>en<br />
wesentlichen E<strong>in</strong>fluss auf den therapeutischen Effekt darstellen.<br />
Bei Patienten mit MDD (Major Depressive Disor<strong>der</strong>) wurde<br />
e<strong>in</strong>e hyperstabile, rigide Vigilanzregulation, d.h. e<strong>in</strong> langes Verbleiben<br />
<strong>in</strong> hohen Vigilanzstadien (A-Stadien mit hoher Alpha-Power-<br />
Fraktion) beschrieben (Ulrich 1994). Ziel <strong>der</strong> Studie ist die Untersuchung<br />
möglicher Zusammenhänge zwischen <strong>der</strong> Stabilität <strong>der</strong><br />
Vigilanzregulation, körperlicher Aktivität und kl<strong>in</strong>ischen Ansprechen<br />
auf SE bei Patienten mit MDD. Angenommen wurde, dass e<strong>in</strong><br />
Ansprechen auf SE mit e<strong>in</strong>er hyperstabilieren Vigilanzregulation<br />
vor SE assoziiert ist, die sich SE stabilisiert.<br />
Methode: Bei 23 stationären Patienten mit MDD <strong>der</strong> Kl<strong>in</strong>ik und<br />
Polikl<strong>in</strong>ik für Psychiatrie des UKL wurde e<strong>in</strong> partieller therapeutischer<br />
SE über 22 Stunden ab 01:00 Uhr durchgeführt. Es erfolgte<br />
e<strong>in</strong>e Bestimmung <strong>der</strong> Vigilanzregulation mittels Ruhe-EEG am<br />
Morgen vor SE, am Morgen nach SE und am nächsten Morgen<br />
nach Schlaf. Die körperliche Aktivität wurde mittels e<strong>in</strong>es Aktometers<br />
über die Gesamtdauer <strong>der</strong> drei Tage aufgezeichnet. E<strong>in</strong><br />
