Prospektive Studien haben zahlreiche Risikofaktoren identifiziert,<br />
die mit dem späteren Auftreten e<strong>in</strong>er schizophrenen Psychose assoziiert<br />
s<strong>in</strong>d. Genetische und umweltbezogene Ursachenfaktoren bewirken<br />
frühe kognitive, Verhaltens- und Erlebnisän<strong>der</strong>ungen bei<br />
später Erkrankenden, die im K<strong>in</strong>desalter meist diskret und diagnostisch<br />
unspezifisch s<strong>in</strong>d, <strong>in</strong> <strong>der</strong> sogenannten Prodromalphase<br />
aber an Intensität zunehmen und sich auch qualitativ verän<strong>der</strong>n<br />
mit dem Auftreten erster positiver psychopathologischer Symptome.<br />
Nach e<strong>in</strong>em kurzen Überblick über neuropsychologische und<br />
psychopathologische Risikofaktoren <strong>in</strong> verschiedenen Altersabschnitten<br />
wird <strong>der</strong> Symposiumsbeitrag Ergebnisse aus dem Früherkennungs-<br />
und Früh<strong>in</strong>terventionsprojekt des Kompentenznetzes<br />
Schizophrenie darstellen und die Frage diskutieren, welche <strong>in</strong>dividuellen<br />
Faktoren e<strong>in</strong>e kl<strong>in</strong>ische Prädiktion des weiteren Verlaufs<br />
ermöglichen. Im Rahmen des Kompetenznetzes Schizophrenie<br />
wurden Rat suchende Patienten anhand psychopathologischer<br />
Symptome klassifiziert als (hypothetisch) psychoseferne Prodrome<br />
(die z. B. Wahrnehmungsverän<strong>der</strong>ungen und Antriebsstörungen<br />
aufweisen) bzw. psychosenahe Prodrome (z. B. bei zeitlich begrenzten<br />
halluz<strong>in</strong>atorische Erlebnissen) und mit e<strong>in</strong>er neuropsychologischen<br />
Testbatterie, teilweise auch mit bildgebenden Verfahren<br />
untersucht. Normabweichungen <strong>in</strong> diesen Verfahren sowie psychopathologische<br />
Symptome wurden <strong>in</strong> Regressionsanalysen als Prädiktoren<br />
e<strong>in</strong>er späteren Erkrankung analysiert. Wie erwartet, prädiziert<br />
ist die <strong>in</strong>itiale Ausprägung von positiver und auch von<br />
desorganisierter Symptomatik (SOPS, Scale for the Assessment of<br />
Prodromal Symptoms) e<strong>in</strong> erhöhtes Erkrankungsrisiko <strong>in</strong> den<br />
nachfolgenden 24 Monaten. Auch neuropsychologische Bee<strong>in</strong>trächtigungen,<br />
vor allem im Bereich des Gedächtnisses, leisten e<strong>in</strong>en<br />
signifikanten Beitrag zur Vorhersage, <strong>der</strong> aber nur <strong>in</strong> <strong>der</strong> Gruppe<br />
<strong>der</strong> psychosefernen Prodrome unabhängig von <strong>der</strong> Prädiktion<br />
durch psychopathologische Merkmale ist. Kl<strong>in</strong>isch ist dies <strong>in</strong>sofern<br />
bedeutsam, als die diagnostische und prognostische Unsicherheit<br />
bei psychosefernen Prodromen hoch ist (nur etwa 10 % erkranken<br />
<strong>in</strong>enrhalb von 24 Monaten) und durch die H<strong>in</strong>zunahme neuropsychologischer<br />
Befunde verm<strong>in</strong><strong>der</strong>t werden kann. Neurobiologisch<br />
lässt sich vermuten, dass e<strong>in</strong>e mediotemporale Dysfunktion <strong>in</strong> <strong>der</strong><br />
frühen Prodromalphase zunächst noch kompensiert werden kann,<br />
und dass <strong>der</strong> Zusammenbruch dieser funktionellen Kompensation<br />
den nahenden Beg<strong>in</strong>n psychotischen Erlebens markiert.<br />

Introduction: Research <strong>in</strong>to the molecular biological basis of psychosis<br />
(schizophrenia and bipolar disor<strong>der</strong>) has reached an important<br />
po<strong>in</strong>t <strong>in</strong> time. Genome-wide association studies (GWAS) have<br />
been performed, encompass<strong>in</strong>g several thousands of samples,<br />
which are analyzed jo<strong>in</strong>tly <strong>in</strong> meta- and megaanalyses. GWAS have<br />
identified several novel susceptibility genes for schizophrenia (SZ)<br />
and bipolar disor<strong>der</strong> (BD). However, variants so far identified account<br />
only for a fraction of disease liability. Thus, GWAS based on<br />
s<strong>in</strong>gle nu cleotide polymorphisms (SNPs) have to be embedded <strong>in</strong> a<br />
framework of complementary approaches, the study of candidate<br />
genes and of other sources of variation (e. g. copy number variations)<br />
as well as of gene-environment <strong>in</strong>teractions, pharmacogenetics,<br />
epi genomics, imag<strong>in</strong>g, neurobiology, and statistical model<strong>in</strong>g.<br />
Method: One largely untraveled avenue so far is the study of longitud<strong>in</strong>al<br />
phenotypes. Until now, world-wide efforts have focused on<br />
cross-sectional samples of categorical cl<strong>in</strong>ical diagnoses of<br />
ICD-10 or DSM-IV-def<strong>in</strong>ed SZ or BD, although it is common cl<strong>in</strong>ical<br />
knowledge that patients differ widely <strong>in</strong> several aspects of the<br />
course of their illness. These <strong>in</strong>clude <strong>in</strong>dividual patterns of relapse,<br />
rega<strong>in</strong> of function<strong>in</strong>g after an acute episode of illness, level of disability,<br />
cognitive function<strong>in</strong>g <strong>in</strong> relation to the duration of illness<br />
among others.<br />
Discussion / Results: A framework is presented that aims at<br />
accommodat<strong>in</strong>g the aforementioned approaches.<br />

Similarities and Differences Between Bipolar Disor<strong>der</strong> and<br />
Schizophrenia with Emphasis on the Early Phases of the Illness<br />

Introduction: Schizophrenia and bipolar disor<strong>der</strong> belong to the<br />
most severe mental disor<strong>der</strong>s that share many similarities, e. g. a<br />
lifetime prevalence of about 1 %. Both disor<strong>der</strong>s are associated with<br />
a recurrent, chronic course, <strong>in</strong>sufficient cl<strong>in</strong>ical response, and functional<br />
disability <strong>in</strong> a substantial number of patients. Furthermore,<br />
both disor<strong>der</strong>s have their typical onset early <strong>in</strong> life (>50 % of patients<br />
report their illness onset prior age of 19) and there is empirical<br />
evidence for a long undetected early course. A lag between symptom<br />
onset and first diagnosis and treatment lasts typically several<br />
years, therefore, a significant functional impairment ren<strong>der</strong>s early<br />
identification and <strong>in</strong>tervention a vital role.
