Differentialdiagnose bipolarer Störungen bei K<strong>in</strong><strong>der</strong>n und Jugendlichen:<br />
Was sagt uns die Forschung für die Praxis?<br />

E<strong>in</strong>leitung: Obwohl erste Kranheitszeichen sich bei e<strong>in</strong>em Großteil<br />
<strong>der</strong> Patienten bereits zwischen dem 15. und 19 Lebensjahr manifestieren,<br />
werden bipolare Störungen häufig erst viel später als solche<br />
richtig diagnostiziert. Für den K<strong>in</strong><strong>der</strong>- und Jugendpsychiater stellt<br />
die differenzialdiagnostische Abgrenzung daher e<strong>in</strong>e beson<strong>der</strong>e<br />
Herausfor<strong>der</strong>ung dar.<br />
Methode: Der Vortrag gibt auf <strong>der</strong> Grundlage <strong>der</strong> verfügbaren Forschungsbefunde<br />
e<strong>in</strong>en Überblick über aktuelle Entwicklungen <strong>in</strong><br />
<strong>der</strong> Differentialdiagnose bipolarer Störungen bei K<strong>in</strong><strong>der</strong>n und<br />
Jugendlichen. Dargestellt werden Daten zu möglichen Vorläufer-<br />
<strong>Erkrankungen</strong>, Überlappungen mit / Unterschieden zu an<strong>der</strong>en<br />
Störungen, Genetik, Komorbidität und Langzeitverlauf.<br />
Diskussion / Ergebnisse: In Deutschland werden bipolare Störungen<br />
vor <strong>der</strong> Adoleszenz praktisch kaum diagnostiziert, während die<br />
Zahlen bipolarer M<strong>in</strong><strong>der</strong>jähriger <strong>in</strong> den USA rasant steigen. Umstritten<br />
ist, ob Irritabilität, schwere Dysregulation von Affekt und<br />
Verhalten („severe mood dysregulation“) und zyklothymes Temperament<br />
im K<strong>in</strong>des- und Jugendalter e<strong>in</strong> erhöhtes Risiko für den<br />
Übergang <strong>in</strong> e<strong>in</strong>e bipolare Störung und damit geeignete Indikatoren<br />
<strong>in</strong> <strong>der</strong> Früherkennung darstellen. E<strong>in</strong>e Schlüsselrolle kommt<br />
dem Erkennen hypomaner Symptome als mögliche Vorläufer bipolarer<br />
Störungen zu, die bei Jugendlichen mit depressiver Erkrankung<br />
aber auch bei externalisierenden Störungen viel zu oft unerkannt<br />
bleiben. Eigene Ergebnisse unterstreichen zudem den engen<br />
Zusammenhang von Substanzmissbrauch mit hypomanen Phasen<br />
bei Adoleszenten. Populationsbasierte genetische Studien zeigen<br />
e<strong>in</strong>e Überlappung genetischer Determ<strong>in</strong>anten von bipolaren Störungen,<br />
ADHS und Schizophrenie. Frühsymptome bipolarer Störungen<br />
und ihre Geme<strong>in</strong>samkeiten und Beson<strong>der</strong>heiten im Vergleich<br />
mit schizophrenen Prodromi bedürfen daher verstärkter<br />
Beachtung. Bei <strong>der</strong> Diagnostik bipolarer Störungen bei Jugendlichen<br />
und K<strong>in</strong><strong>der</strong>n sollte weiterh<strong>in</strong> vorrangig auf das Auftreten von<br />
abgrenzbaren Episoden mit e<strong>in</strong>deutigen Stimmungsän<strong>der</strong>ungen<br />
und begleitenden Verän<strong>der</strong>ungen von Kognition und Verhalten geachtet<br />
werden, die am ehesten e<strong>in</strong>e Unterscheidung von ADHS,<br />
depressiven Episoden und schizophrenen Störungen erlauben.<br />

Introduction: Precise specification of neural mechanisms of antidepressant<br />
medication <strong>in</strong> treatment of Major Depressive Disor<strong>der</strong><br />
(MDD) and search<strong>in</strong>g for neurobiological markers for treatment<br />
outcome are both relevant for better un<strong>der</strong>stand<strong>in</strong>g and improvement<br />
of treatment algorithms. Us<strong>in</strong>g fMRI differential effects of<br />
serotonergic and noradrenergic antidepressants on bra<strong>in</strong> activation<br />
were <strong>in</strong>vestigated dur<strong>in</strong>g a cognitive control task. Functional bra<strong>in</strong><br />
parameters were obta<strong>in</strong>ed to predict treatment response particularly<br />
with regard to the pretreatment hyperactivity <strong>in</strong> the rostral anterior<br />
c<strong>in</strong>gulate cortex (rACC).<br />
Method: Twenty patients un<strong>der</strong>went a naturalistic open-label cl<strong>in</strong>ical<br />
treatment with either the serotonergic antidepressant citalopram<br />
(n=12) or the noradrenergic antidepressant rebox-et<strong>in</strong>e<br />
(n=8). FMRI was performed at basel<strong>in</strong>e and after 6 weeks of the<br />
cl<strong>in</strong>ical treatment course.<br />
Discussion / Results: There were no significant differences <strong>in</strong> cl<strong>in</strong>ical<br />
characteristics, treatment outcome and <strong>in</strong> basel<strong>in</strong>e fMRI activation<br />
between both medication groups. GROUP by TIME <strong>in</strong>teraction<br />
revealed significant voxels <strong>in</strong> the right amygdala-hippocampus<br />
complex <strong>in</strong>dicat<strong>in</strong>g a strong BOLD signal decrease <strong>in</strong> the citalopram<br />
group after treatment. No significant relationship was detected<br />
between pretreatment rACC hyperactivity and relative symptom<br />
improvements. These results strongly <strong>in</strong>dicate that serotonergic<br />
and noradrenergic antidepressants have a differential effect on<br />
bra<strong>in</strong> activity, especially on amygdala-hippocampus activity.<br />

Introduction: The efficacy of different pharmacological ma<strong>in</strong>tenance<br />
therapies <strong>in</strong> reduc<strong>in</strong>g recurrences has been widely documented.<br />
However, <strong>in</strong> cl<strong>in</strong>ical practice, long-term pharmacotherapy<br />
might be associated with prolonged side effects and discont<strong>in</strong>uation<br />
of medication which un<strong>der</strong>m<strong>in</strong>e its effectiveness. Whereas<br />
pharmacological treatments aim at reduc<strong>in</strong>g depressive symptoms<br />
on a biological level, cognitive behavior therapy aims at chang<strong>in</strong>g<br />
psychosocial vulnerabilities that trigger recurrence of depressive<br />
episodes, such as dysfunctional cognitive styles and rum<strong>in</strong>at<strong>in</strong>g.<br />
Although the benefit of cognitive behavior therapy appears plausible,<br />
only few studies have <strong>in</strong>vestigated its efficacy <strong>in</strong> recurrent depression<br />
until now.<br />
Method: The major goal of the multi-centre, controlled, randomised<br />
trial is to compare the long-term outcome of cognitive-behavioural<br />
ma<strong>in</strong>tenance therapy (CBMT) plus pharmacological cont<strong>in</strong>uation/ma<strong>in</strong>tenance<br />
treatment (treatment as usual, TAU) versus<br />
manualized psychoeducation (MAPE) plus TAU for out-patients<br />
with recurrent depression. Patients meet<strong>in</strong>g the diagnosis of recurrent<br />
depressive disor<strong>der</strong> (³ 3 major depressive episodes, MDE), currently<br />
<strong>in</strong> remission and exclusion criteria were randomly assigned<br />
to one of the two conditions (CBMT plus TAU or MAPE plus TAU).<br />
186 patients have been recruited from 14 outpatient cl<strong>in</strong>ics of psychiatric<br />
hospitals and psychological departments <strong>in</strong> the Rh<strong>in</strong>e-<br />
Ma<strong>in</strong> region and Thur<strong>in</strong>gia. The primary outcome measure is time<br />
to first relapse/recurrence, assessed by the LIFE / SCID (Longitud<strong>in</strong>al<br />
Interval Follow-up Evaluation). This will by analyzed us<strong>in</strong>g a<br />
Cox proportional hazards regression model us<strong>in</strong>g <strong>in</strong>tervention<br />
group and severity of <strong>in</strong>dex MDE as predictors.<br />
Discussion / Results: Data are currently analysed and will be presented<br />
on the congress.<br />
