Introduction: Biological markers of the at-risk mental state for<br />
psychosis (ARMS) are crucial for early recognition and therapeutic<br />
<strong>in</strong>tervention <strong>in</strong> ultra-high risk <strong>in</strong>dividuals. In this regard, previous<br />
studies showed that the ARMS is associated with subtle neuroanatomical<br />
abnormalities found <strong>in</strong> similar bra<strong>in</strong> regions as <strong>in</strong> the established<br />
disor<strong>der</strong>. We employed multivariate analysis techniques <strong>in</strong><br />
or<strong>der</strong> to <strong>in</strong>vestigate whether different ARMS for psychosis and their<br />
cl<strong>in</strong>ical outcomes could be reliably diagnosed on the <strong>in</strong>dividual<br />
level based on structural bra<strong>in</strong> alterations.<br />
Method: First, a multivariate, multi-group support-vector mach<strong>in</strong>e<br />
(SVM) classification analysis was performed on the structural magnetic<br />
resonance imag<strong>in</strong>g (MRI) data of <strong>in</strong>dividuals <strong>in</strong> early (n=24),<br />
late (n=27) ARMS of psychosis and healthy controls (HC, n=25).<br />
Then, the method’s ability <strong>in</strong> predict<strong>in</strong>g subsequent transitions to<br />
psychosis based on the basel<strong>in</strong>e MRI data was evaluated <strong>in</strong> a subgroup<br />
of the ARMS population with available cl<strong>in</strong>ical follow-up<br />
<strong>in</strong>formation (transitions: n=16, non-transitions: n=18) compared<br />
to HC (n=17). The specificity, sensitivity, accuracy, significance and<br />
generalizability of the methodology were evaluated by means of<br />
permutation analysis and five-fold cross-validation.<br />
Discussion / Results: The 3-group, cross-validated classification accuracies<br />
of the first analysis were 86 % (HC vs the rest), 91 % (early<br />
at-risk <strong>in</strong>dividuals vs the rest), and 86 % (late at-risk <strong>in</strong>dividuals vs<br />
the rest). The accuracies <strong>in</strong> the second analysis were 90 % (HC vs<br />
the rest), 88 % (<strong>in</strong>dividuals with transition vs the rest), and 86 %<br />
(<strong>in</strong>dividuals without transition vs the rest). These f<strong>in</strong>d<strong>in</strong>gs suggest<br />
that different ARMS and their cl<strong>in</strong>ical outcomes may be reliably<br />
identified on an <strong>in</strong>dividual basis by assess<strong>in</strong>g patterns of wholebra<strong>in</strong><br />
neuroanatomical abnormalities.<br />

Abnormal prefrontal activation is directly related to pre-synaptic<br />
striatal dopam<strong>in</strong>e dysfunction <strong>in</strong> people at cl<strong>in</strong>ical risk for psychosis<br />

The pathophysiology of schizophrenia is <strong>in</strong>completely un<strong>der</strong>stood,<br />
but two of the most robust abnormalities are elevated striatal dopam<strong>in</strong>e<br />
activity and prefrontal cortical dysfunction. To <strong>in</strong>vestigate the<br />
relationship between these abnormalities <strong>in</strong> the prodromal phase of<br />
the illness, we comb<strong>in</strong>ed functional Magnetic Resonance Imag<strong>in</strong>g<br />
and 18F-Dopa Positron Emission Tomography. When perform<strong>in</strong>g<br />
a verbal fluency task, subjects with an At Risk Mental State showed<br />
greater activation <strong>in</strong> the <strong>in</strong>ferior frontal cortex than controls. Striatal<br />
dopam<strong>in</strong>e function was greater <strong>in</strong> the At Risk group than <strong>in</strong> controls.<br />
With<strong>in</strong> the At Risk group, but not the control group, there was<br />
a direct correlation between the degree of left <strong>in</strong>ferior frontal activation<br />
and the level of striatal dopam<strong>in</strong>e function. The key f<strong>in</strong>d<strong>in</strong>g<br />
from the present study is that <strong>in</strong> <strong>in</strong>dividuals at very high risk of<br />
schizophrenia, altered prefrontal activation dur<strong>in</strong>g a task of executive<br />
function was directly related to striatal hyperdopam<strong>in</strong>ergia.<br />
This provides <strong>in</strong> vivo evidence of a l<strong>in</strong>k between dopam<strong>in</strong>e dys-<br />
function and the perturbed prefrontal function which may un<strong>der</strong>lie<br />
the deficits <strong>in</strong> executive process<strong>in</strong>g evident <strong>in</strong> people with prodromal<br />
symptoms of psychosis. These abnormalities reflect an <strong>in</strong>creased<br />
vulnerability to psychosis and predate the first episode of<br />
frank psychosis.<br />

Introduction: Schizophrenie-Spektrums-Störungen – Cluster A- Persön<br />
lichkeitsstörungen (PS) nach DSM-IV und <strong>in</strong>sbeson<strong>der</strong>e die<br />
schizotypische PS – fanden sich gehäuft bei Personen mit e<strong>in</strong>em<br />
erhöhten symptomatisch def<strong>in</strong>ierten Risiko für die Entwicklung<br />
e<strong>in</strong>er ersten psychotischen Episode. Dies überrascht zunächst aufgrund<br />
des phänomenologischen Überschneidungsbereichs <strong>in</strong>sbeson<strong>der</strong>e<br />
<strong>der</strong> attenuierten psychotischen Symptome (APS) <strong>der</strong><br />
‚ultra-high risk’-Kriterien mit Kriterien zur Diagnose <strong>der</strong> schizotypischen<br />
PS nicht.<br />
Methode: Zur Klärung, ob Persönlichkeitsakzentuierungen (PA)<br />
o<strong>der</strong> auch PS geeignet se<strong>in</strong> können, zwischen kl<strong>in</strong>isch def<strong>in</strong>ierten<br />
Risikopersonen mit (N=50) und ohne zwischenzeitlichen Übergang<br />
<strong>in</strong> e<strong>in</strong>e Psychose (N=50) zu unterscheiden, wurden diese anhand<br />
e<strong>in</strong>er Selbstbeurteilungsskala für PS verglichen. Die Gruppen<br />
waren h<strong>in</strong>sichtlich ihrer Risikokriterien (psychosefernes und psychosenahes<br />
Prodrom) sowie Geschlecht und Alter parallelisiert.<br />
Diskussion / Ergebnisse: Insgesamt berichteten Personen mit späterer<br />
Entwicklung e<strong>in</strong>er Psychose tendenziell mehr PS und ausgeprägtere<br />
PA. H<strong>in</strong>sichtlich <strong>der</strong> <strong>in</strong>sgesamt seltenen PS wurde nur die<br />
depressive subaffektive PS nach DSM-IV signifikant; sie fand sich<br />
bei 58 % <strong>der</strong> Patienten mit und 34 % <strong>der</strong>er ohne Psychoseentwicklung.<br />
Zudem zeigte sich e<strong>in</strong> Trend für e<strong>in</strong>e häufigeres Auftreten<br />
m<strong>in</strong>destens e<strong>in</strong>er Cluster-A-PS <strong>in</strong> den ‚echten Prodromen’ (20 % vs.<br />
8 %). E<strong>in</strong> Vergleich <strong>der</strong> PA zeigte entsprechend signifikant höhere<br />
Ausprägungen <strong>der</strong> Personen mit Übergang auf <strong>der</strong> depressiven und<br />
Cluster-A sowie <strong>der</strong> schizoiden Dimension und zudem e<strong>in</strong>en statistischen<br />
Trend für die schizotypische und asthenische Dimension.<br />
E<strong>in</strong>e weitere Betrachtung <strong>der</strong> PA ergab, dass dabei ausschließlich<br />
die schizoide PA als möglicher Prädiktor e<strong>in</strong>er Psychoseentwicklung<br />
bei Risikopersonen <strong>in</strong> Frage zu kommen sche<strong>in</strong>t. Die <strong>in</strong> <strong>der</strong><br />
ausgeprägteren Selbste<strong>in</strong>schätzung <strong>der</strong> Personen mit Psychoseentwicklung<br />
im Beobachtungszeitraum auf <strong>der</strong> schizoiden Dimension<br />
zutrage tretende schlechtere soziale E<strong>in</strong>b<strong>in</strong>dung wird gestützt von<br />
Befunden <strong>der</strong> genetischen Hochrisikoforschung zu bereits prämorbid<br />
bestehenden sozialen Defiziten bei K<strong>in</strong><strong>der</strong>n mit schizophrenem<br />
Elternteil und unterstreicht auch noch e<strong>in</strong>mal den Stellenwert e<strong>in</strong>er<br />
guten sozialen E<strong>in</strong>b<strong>in</strong>dung als potentiellen Schutzfaktor.<br />
