Challenging times for mental health services

Many countries are in the middle of comprehensive reforms of their
mental health systems that require funding. At the same time, they
are affected by the consequences of the financial crisis. Governments
have come up with multi-billion dollar rescue packages.
Countries need to make cuts in public sector expenditure. At an
individual level, debt status is already high in many countries due to
falling house prices and high consumption levels. The crisis has several
consequences for the mental health of the population. Loss of
employment and risk of unemployment are associated with an increase
of stress, anxiety, depression and psychotic disorders. For
people with severe mental disorders, the increase of unemployment
increases competition for placements in protected workplaces. These
social and economic developments are taking place at a point in
time when mental health services are focussing on the development
of community services targeting vulnerable groups. However, a
particular challenge at international level is the variation of existing
provisions and use of resources, often poorly placed to deal with the
consequences of the economic crisis, and many governments are
facing the need to reduce expenditure. A priority is therefore to
protect essential community services for the most vulnerable and
advocate for the development of additional effective interventions
promoting independence and social inclusion. There is place for
some optimism, since increasing number of countries are committed
to mental health reform, which now has to be accelerated and
sustained. This will prove to be a crucial test for mental health, but
the increasing effectiveness of international partnerships that are
raising awareness gives hope.

The German Alliance for Mental Health and its role for occupational

A. Statz
The German Alliance for Mental Health has been founded in 2004
to build a network of partners in the field of mental health to jointly
promote the public awareness of mental health and ill health and
to fight the stigma of mental illness. It has been founded by the
German Psychiatry and Psychotherapy Association (DGPPN) and
“Open the doors”, the German branch of the worldwide World Psychiatric
Association programme against stigma and discrimination
because of schizophrenia and is supported by the German Federal
Ministry of Health. Until now, it has developed to a nationwide
open network of more than 50 professional associations in the field
of mental health, research networks, and self-help associations of
service users and relatives. Mental health and well-being at the
workplace has been identified as an important starting point for
promoting social integration of people with mental illness. Employment
is usually a strong psychosocial resource, nevertheless
mental health problems are very common at workplaces being on a
steady rise in recent years, e. g. in terms of illness-related absenteeism
or early retirement. Additionally, the stigma of mental illness
hampers early recognition of mental illness as well as the reintegration
of people with mental illness after illness-related absences
(e. g. after hospital or rehabilitation stays). To raise awareness and to
find new ways for a better integration of people with mental illness
in working environments, the German Alliance for Mental Health
has organized two high-ranking symposia in 2009, i) together with
the EC DG Health & Consumers, WHO Europe and the German
Federal Ministry of Health, and ii) together with the German Medical
Association. Main themes were the reduction of stigma of
mental illness as well as mental health promotion at the workplace,
and strategies for reintegration of people with mental illness into
work. Conclusions of the symposia and resulting plans for future
activities will be presented and discussed.
