tion zwischen FSKL Score und realer Leistung. Bei den depressiven<br />
Patienten korrelierte h<strong>in</strong>gegen die subjektive Beurteilung signifikant<br />
mit verschiedenen Testleistungen. Betroffen waren überwiegend<br />
exekutive Funktionen wie <strong>der</strong> Subtest Intra / Extra Dimensional<br />
Set-Shift <strong>der</strong> Cambridge Neuropsychological Test Automated<br />
Battery (CANTAB; R=0,28; p=0,005), Reaktionsflexibilität (NoGo-<br />
Paradigma; R=0,34; p=0,0006), sensomotorische Interferenz<br />
(R=0,21; p=0,04) und visuomotorisches Pursuit Track<strong>in</strong>g (R=0,27;<br />
p=0,006) aber auch die Daueraufmerksamkeitsleistung (-Paradigma;<br />
R=0,21; p=0,03). Die Profile von subjektiver Leistungse<strong>in</strong>schränkung<br />
und tatsächlicher Testergebnissen ließen sich bezüglich<br />
<strong>der</strong> betroffenen Leistungsbereiche nicht überzeugend zur<br />
Deckung br<strong>in</strong>gen. Auch aus diesem Grund muss offen bleiben, ob<br />
die sensible depressive Selbstwahrnehmung lediglich leichte und<br />
statistisch im Gruppenvergleich nicht e<strong>in</strong>deutig nachweisbare kognitive<br />
Bee<strong>in</strong>trächtigungen registriert, o<strong>der</strong> ob die Korrelationen<br />
durch e<strong>in</strong>en hemmenden E<strong>in</strong>fluss <strong>der</strong> negativen Leistungserwartung<br />
auf die (exekutive) Testleistung. Weitere Untersuchungen s<strong>in</strong>d<br />
erfor<strong>der</strong>lich, um diese Frage zu klären.<br />

Introduction: The norep<strong>in</strong>ephr<strong>in</strong>e (NET) and seroton<strong>in</strong> (5-HTT)<br />
transporter genes constitute promis<strong>in</strong>g candidate genes <strong>in</strong> Major<br />
Depression. A comprehensive set of tagg<strong>in</strong>g SNPs cover<strong>in</strong>g the<br />
NET gene region as well as 5-HTTLPR and 5-HTT rs25531 polymorphisms<br />
were analyzed with respect to antidepressant treatment<br />
response with particular attention gen<strong>der</strong>-effects and subtypes of<br />
melancholic or anxious depression.<br />
Method: 252 unrelated Caucasian patients (f=142; m=110) with<br />
Major Depression were genotyped for NET and 5-HTT polymorphisms.<br />
Genotype effects on HAM-D change scores over six weeks<br />
of antidepressant treatment were analyzed us<strong>in</strong>g ANOVA with<br />
repeated measures. There was no effect of any of the <strong>in</strong>vestigated<br />
eight NET or two 5-HTT polymorphisms on overall treatment response.<br />
However, stratification for anxious versus non-anxious depression<br />
revealed a significantly detrimental effect of the less active<br />
5-HTTLPR S allele (p=0.007) and 5-HTTLPR / 5-HTT rs25531<br />
haplotypes on treatment response <strong>in</strong> patients with anxious depression.<br />
Discussion / Results: The present f<strong>in</strong>d<strong>in</strong>gs do not support a major<br />
impact of NET and 5-HTT genes on antidepressant treatment response<br />
<strong>in</strong> Major Depression per se. The observed significant <strong>in</strong>fluence<br />
of 5-HTT gene variation on antidepressant treatment <strong>in</strong><br />
anxious depression, however, po<strong>in</strong>ts to anxious depression as a potential<br />
diagnostic entity of its own requir<strong>in</strong>g specific diagnostic and<br />
therapeutic attention.<br />

Introduction: Several studies reported a decreased pa<strong>in</strong> sensitivity<br />
<strong>in</strong> patients with depression, but the un<strong>der</strong>ly<strong>in</strong>g neurobiological mechanisms<br />
of this phenomenon are unclear. While there is extensive<br />
evidence that the seroton<strong>in</strong>ergic system plays a key role <strong>in</strong> pa<strong>in</strong> modulation,<br />
especially <strong>in</strong> pa<strong>in</strong> <strong>in</strong>hibitory mechanisms via descend<strong>in</strong>g<br />
pathways, as well as <strong>in</strong> the pathophysiology of depression, no study<br />
so far has exam<strong>in</strong>ed its potential relevance <strong>in</strong> mediat<strong>in</strong>g the alteration<br />
of pa<strong>in</strong> process<strong>in</strong>g. The present study addresses the question of<br />
whether <strong>in</strong>dices of seroton<strong>in</strong>ergic dysfunction, as <strong>in</strong>vestigated by a<br />
neuroendroc<strong>in</strong>e challenge paradigm, are related to pa<strong>in</strong> sensitivity.<br />
Method: N<strong>in</strong>eteen drug-free <strong>in</strong>patients with unipolar major depression<br />
un<strong>der</strong>went a neuroendocr<strong>in</strong>e challenge test by measur<strong>in</strong>g<br />
cortisol and prolact<strong>in</strong> <strong>in</strong> response to <strong>in</strong>travenously adm<strong>in</strong>istered<br />
clomipram<strong>in</strong>e (12.5 mg). Heat / cold pa<strong>in</strong> thresholds, warmth / cold<br />
detection thresholds, measures of current pa<strong>in</strong> compla<strong>in</strong>ts and<br />
mood were assessed the day before and three day after challenge<br />
procedure.<br />
Discussion / Results: When patients were classified <strong>in</strong> subgroups<br />
based on a median split of their cortisol response values, the lowresponsive<br />
group showed significantly elevated heat pa<strong>in</strong> thresholds<br />
and nearly significantly elevated cold pa<strong>in</strong> thresholds compared to<br />
the high-responsive group. No such group differences were found<br />
with regard to somatosensory thresholds, measures of pa<strong>in</strong> compla<strong>in</strong>ts<br />
and mood. Subgroup<strong>in</strong>g on the basis of prolact<strong>in</strong> responsiveness<br />
did not reveal significant differences <strong>in</strong> any parameter. In summary,<br />
a decreased pa<strong>in</strong> sensitivity was demonstrated <strong>in</strong> patients<br />
characterized by a reduced neuroendocr<strong>in</strong>e responsiveness to clomipram<strong>in</strong>e,<br />
suggest<strong>in</strong>g an <strong>in</strong>volvement of seroton<strong>in</strong>ergic dysfunction<br />
un<strong>der</strong>ly<strong>in</strong>g altered pa<strong>in</strong> perception <strong>in</strong> depression.<br />

E<strong>in</strong>leitung: Funktionell-magnetresonanztomographische (fMRT)<br />
Untersuchungen von Patienten mit bipolar affektiver Störung zeigten<br />
konsistent e<strong>in</strong>e Hyperaktivität von Hirnarealen, die affektiven<br />
Verarbeitungsprozessen zugrunde liegen. Im Gegensatz dazu ist die<br />
Literatur <strong>in</strong> Bezug auf verän<strong>der</strong>te Aktivierungsmuster <strong>in</strong> den präfrontalen<br />
Kortizes weniger e<strong>in</strong>heitlich.<br />
Methode: Um Klarheit über mögliche Fehlfunktionen <strong>in</strong> Subregionen<br />
des präfrontalen Kortex zu erlangen, führten wir e<strong>in</strong>e Metaanalyse<br />
mit „activation likelihood estimation“ (ALE) durch. Dazu<br />
wurden im „Web of Science“ und <strong>in</strong> „MEDLINE“ fMRT-Studien<br />
mit bipolaren Patienten recherchiert. Alle Studien, die signifikante<br />
Aktivitätsunterschiede <strong>in</strong> präfrontalen Kortizes bipolarer Patienten<br />
<strong>in</strong> Talairach- o<strong>der</strong> MNI-Koord<strong>in</strong>aten angaben, wurden e<strong>in</strong>geschlossen.<br />
Diskussion / Ergebnisse: In <strong>der</strong> ALE-Metaanalyse zeigten sich signifikante<br />
Hyperaktivität im Broca-Areal und im anterioren Anteil<br />
des rechten Gyrus frontalis medius sowie signifikante Hypoaktivität<br />
bilateral im orbitofrontalen Kortex. E<strong>in</strong>e weitere Analyse <strong>der</strong><br />
e<strong>in</strong>geschlossenen Studien ergab, dass die lateral-präfrontale Hyperaktivität<br />
ausschließlich während Arbeitsgedächtnis- und Verbalisierungsaufgaben<br />
